  • Physical Education Waiver

  • Senate Bill 610 requires that Kindergarten-5th grade learners participate in physical education 150 minutes each week. It also requires the equivalent of one class period per day of physical education for one semester each year for learners in grades 6-8.

    I acknowledge and agree that I am choosing to enroll/re-enroll my learner in a school of choice which offers several arts enhancement and elective courses including, but not limited to: art, music, technology, theater and dance, in addition to physical education.

    Should my child be scheduled for less than the required minutes of physical education per week, I agree that my learner will participate in physical activities outside the school day, which are equal to or in excess of the State of Florida's mandated requirement of physical education, unless they are medically exempt from doing so.

  • I, as a parent/guardian of {name54}, enter into a contract agreement with Dayspring Academy upon the date of this submission to create an atmosphere of success for my child's educational journey at Dayspring. I/we, the undersigned, fully understand that the teachers and staff alone are not responsible for my child's learning. I/we pledge to invest the time and effort necessary to ensure my child's education is one of excellence by:

    1. Supporting the mission of Dayspring Academy

    2. Supporting policies regarding attendance, code of conduct, school hours, safety, emergency information, change of address/telephone and health concerns as outlined in the Learner Parent Handbook

    3. Supporting the school uniform dress code

    4. Showing respect and support for my child, teacher and school

    5. Openly and positively communicating to the proper person or staff concering the welfare of my child

    6. Assisting my child daily in homework and special projects when applicable

    7. Reinforcing what my child is learning at school in our home by designating reading time, game time, or family time (suggested 30-45 minutes daily)

    8. Monitoring the amount and content of my child's television and electronics viewing

    9. Providing encouragement and assistance to promote good study habits

    10. Providing supplies my child needs as requested by the teacher

    11. Participating in parent/teacher/learner conferences every nine weeks

  • By registering your learner at Dayspring Academy, you are agreeing to actively participate in your learner's education.

    PLEASE NOTE: Dayspring Academy parents are required to commit to 15 family engagement hours between July 1st and June 30th of each school year. There are many opportuntities to volunteer at events throughout the year in classrooms, on field trips, etc.

  • Personal Property Waiver

  • Learners who bring musical instruments and other personal items onto Dayspring Academy campuses are responsible for the security of such items. Learners will also be responsible for the security of personal items when involved in performing arts activities off campus (e.g. performances, contests, festivals, football games, etc.).

    Dayspring Academy is not responsible for the property of others that is placed on campus at any time or other locations during school sponsored events. On-campus storage facilities, such as band rooms and various learner lockers, are provided as a convenience to students.

     

    By my (our) signature below, I (we) understand that Dayspring Academy does not guarantee the safety or security of stored personal property, and I (we) agree to hold the school system harmless in the event of property loss or damage.

  • MyStudent Parent Portal Acknowledgement

  • I understand that MyStudent, powered by FOCUS, can be accessed through the Pasco ONE app or a web browser. I acknowledge that MyStudent is the primary platform for communication between Dayspring Academy and families.

    I understand that it is my responsibility to regularly access the MyStudent Parent Portal or Pasco ONE app to review my learner's grades, attendance, assignments, and school communications.

    I acknowledge that I am required to review and electronically sign the Parent Bill of Rights, Photo Consent, and Emergency Contact Form within the MyStudent Parent Portal.

    I understand and agree that it is my responsibility to keep my learner's information current by updating the Pasco County MyStudent Parent Portal as needed. This includes demographic information, health information, custodial information, emergency contacts, and uploading any required documentation.

    I also acknowledge that changes to my learner's school records cannot be made unless I update the information in the MyStudent Parent Portal. 

    If I am unable to access my learner's account and need my learner's Student ID or my Parent PIN, I understand that I must contact my campus front office or email studentservices@ dayspringacademy.org for assistance.

  • At Dayspring Academy, we empower our learners to explore and excel using modern technology as part of their educational journey. To support this, we offer optional device insurance to help protect against accidental damage.

  • Accepting Insurance*
    I understand this insurance plan covers up to two (2) accidental device breaks during one school year. I understand that any breaks beyond two (2), or any breaks at all that are determined to be due to negligence or abuse of the device are NOT covered under this plan and are my financial responsibility to cover. I understand that this fee is non-refundable. I understand that if payment is not successfully submitted before the end of the first week of school, the device will not be covered and will no longer be eligible for coverage. Insurance does not cover lost or stolen devices without a police report. Insurance does not cover lost charging cords.

    Chromebook Insurance per K-10th Grade Learner: $35.00

    Macbook Insurance per 11 - 12th Grade Learner: $50.00

     

    Declining Insurance
    By declining insurance, I am responsible for ANY damage (including accidental) to the device. I am responsible for all costs incurred for repair or replacement of the device.
